If your credit card has been lost, cancelled or replaced, we will need a letter from the bank stating that.
You can upload the letter via your MyAva account (click here to learn how).
Please make sure the letter contains the following information:
- The Credit card owner's name
- The date of cancellation
- The first and last 4 digits of the old card's* number
Please note: If your old card was replaced with a new one, please make sure to have the first 4 and last 4 digits of the new card as well as the old one.
