If your credit card has been lost, canceled or replaced, we will need a letter from the bank stating that the card has been canceled or replaced.
You can send it to: firstname.lastname@example.org or upload via the website.
Please make sure the letter contains the following information:
- The owner's name
- The date of cancellation
- The first and last four digits of the old card's* number
*In the case that a new card replaces the old card please make sure to have the first four and last four digits of the new card as well.
